Once you submit your request, you will receive an e-mail notifying you that a request for appointment has been made (please make sure to mark the sender as safe if this goes to your spam folder) and an additional e-mail once the appointment has been confirmed by me. At this time, you will also receive an invitation to create an account for my client portal where you will be able to access my intake forms. Please complete these forms as soon as possible so that I may prepare for your visit.
